You're going to get to the "just perjuring themselves" part, right? A lot of the steps above are designed to avoid having any evidence that might impeach the fake story. Not to mention the "protections" that give them a waiting time before testifying to get their story straight, and allowing them to talk with the other witnesses (that last part isn't explicit but seems endemic).

I literally just belly laughed. When there’s so much misconduct people are clamoring for their favorite kind. Yes definitely that’s coming.

This may be the best (most important?) reform you’ve mentioned and yes, dismissal should be the response to “failure to comply” on the “serve & protect” side.
